Architectural Review Request

Download, fill out and submit the AC Review Request form prior to making any changes to the exterior of your home.

If you have a vendor or contractor who offers to submit forms to the LCHA on your behalf, you may have them prepare the form, but it must be signed and submitted by the homeowner.
